ACC case managers can refer clients directly to Enliven by emailing the referral to enlivenreferrals@psn.org.nz.
If you have an ACC claim and would like Enliven to be your Maximising Independence (MI) lead provider, speak with your ACC case manager and request Presbyterian Support Northern – Enliven as your preferred provider.
Your choice matters. ACC clients can express a preference for the provider they would like to support them through their MI journey.

Enliven provides ACC Maximising Independence (MI) services in:
We work with you and others involved in your care
We will tailor a service plan in partnership with you, your whanau and other health providers involved in your rehabilitation. We will then work alongside you to achieve your goals at every stage of your rehabilitation journey.
Our Support Workers are all qualified to at least Level 3 of the New Zealand Certificate in Health and Wellbeing.
Enliven is experienced
We pioneered restorative home-based support in New Zealand in 2004. And have had health professionals on our team before any other home-based support providers.
What happens once a referral is received?
Once we receive an ACC 5972 MI referral, our Enliven Referrals Team checks our current capacity and assesses whether we can accept the referral.
We aim to accept or decline referrals within 24 hours, or sooner where the referral is identified as urgent.
For complex referrals, the acceptance timeframe may be longer while we work through the information and requirements. If there is likely to be a delay, we will keep the ACC case manager informed throughout the process.
Once a referral has been accepted, we advise ACC and progress the referral.
What does ACC require at referral?
To help us assess and action a referral, ACC should provide an ACC 5972 MI referral form, along with any relevant supporting documentation, such as the ACC45 Claim Form and/or ACC7434 Social Rehabilitation Needs Assessment, where applicable.
