Enliven provides home and community based support to disabled people, no matter what level of care they require. Our trained staff develop individually-tailored support plans with clients and their families, giving them choices and involving them in all decisions.
At Enliven Counties Manukau (Auckland South) we are contracted by ACC to provide home and community support to those with injuries.
We are also contracted by the Counties Manukau District Health Board to support older people with low to high needs and provide long term support for those with chronic conditions.
Referrals are received by ACC or the local Needs Assessment and Service Co-ordination (NASC) agency.
