Enliven provides home and community based support to disabled people, no matter what level of care they require. Our trained staff develop individually-tailored support plans with clients and their families, giving them choices and involving them in all decisions.
In Auckland’s West and Northern regions (Waitemata) Enliven is contracted by the Ministry of Health (MOH) to provide reliable and flexible home and community support to people living with a disability. We are also contracted by the Waitemata District Health Board (WDHB) to provide long term support for those with chronic conditions. Referrals are received via ACC or Taikura Trust (the Needs Assessment and Service Co-ordination agency of the Ministry of Health) or the DHB’s local Needs Assessment and Service Co-ordination (NASC) agency.
We run our Individualised and Enhanced Individualised Funding services from here too.
