In Hamilton we are contracted by ACC to provide home and community based support to those with injuries and by the Waikato District Health Board to support older people with low to high needs.

Referrals are received by ACC or the local Needs Assessment and Service Co-ordination (NASC) agency.

Post-discharge Support

Coping on your own again after being in hospital isn’t easy when you’re already living with the everyday challenges of ageing, injury or disability. We’re here to help you in the first four to six weeks after you’ve come out of hospital and to support you on your journey to recovery and independence.
